A parent/legal guardian must complete this form for participating minors. Adults age 18 or older may complete it for themselves. Parents/guardians must remain with and supervise their children; this is not a drop-off event.

Participation is voluntary. Games, movement, crafts and outdoor activities may involve falls, collisions, uneven ground, weather exposure and other inherent risks of injury. Follow staff instructions, use equipment as directed, and stop any activity that feels unsafe.

To the extent permitted by Florida law, I accept these inherent risks and release Lights To Lyfe and its event volunteers from claims arising solely from those inherent risks. This does not release claims for negligence, gross negligence, reckless or intentional misconduct, or rights that cannot lawfully be waived.

In an emergency, staff may contact emergency services and my emergency contact. This form does not authorize non-emergency medical treatment. Photo/video permission is handled separately.
