Holiday Family Resource & Youth Enrichment Fair
Welcome, Lights To Lyfe families! Complete the activity waiver before joining youth activities, make your photo/video choice, and share your feedback after the event.
Parents or legal guardians complete forms for participants under 18. Adults 18 and older may consent for themselves. Parents must stay with and supervise their children.
1. Youth Activity Waiver
Read the activity terms and provide participant details, an adult acknowledgment and an emergency contact. If you decline, speak with staff before joining activities.
Complete Activity Waiver2. Photo & Video Release
Choose whether Lights To Lyfe may use photos/video of your listed participants. Declining media permission does not affect participation or access to resources. Tell staff your choice at check-in.

After the event, tell us what helped your family and what you would like next. Feedback is voluntary; name and email are optional.
